those offers. There is a maximum rebate of (10 units) per end user individual or company for entire promotional
period. Resellers cannot claim this rebate on behalf of customers. Claim documents must be postmarked
within 30 days of the invoice date.

promotion period. A street address is required to receive your rebate and no refunds will be made to a

Purchase:
Eligible products during the specified promotional period from an authorized HP reseller or
direct from HP.
Submit:
a) A completed and signed claim form that is postmarked and received within the
required dates.
b) The original or photocopied invoice(s)/receipt(s) for the specified product(s) and
within the applicable dates. Please circle the purchase price.
c) The serial number(s) and original UPC barcode label(s) for each qualifying product.
Receive:
Your rebate check(s) equal to amount indicated above, to a maximum of 10 units per end user,
individual, or company. Please allow 6 - 8 weeks for processing. If terms and conditions are not
met, rebate will not be sent. HP is not responsible for any claims lost, damaged, or delayed in
transit. Rebate checks are void if not cashed within 90 days of issuance and cannot be
reissued.

To qualify for your rebate(s) you must include original UPC barcode label for each qualifying
product.
UPC/Barcode Label: The HP UPC/barcode label is located on the outside of your HP product
box. Barcode must include serial and product number. The sample label shown is just an
example. Please note that some UPC/barcode labels have up to 7 barcodes.
General Terms and Conditions:
