Corrections/Updates to the
2010 It’s Your Choice: Decision Guide
If you are covered under the State’s Group Health Insurance Program, please note the following
corrections/updates to your 2010 It’s Your Choice: Decision Guide Booklet. The Department of
Employee Trust Funds always strives to provide the most complete and accurate information possible
so that you make informed health insurance coverage decisions. We apologize for any inconvenience

State of Wisconsin Employees, Annuitants and Graduate Assistants:
On page 7, the Online Help chart erroneously describes the program as a video. The second
paragraph should read: “Review ETF’s newest online tutorial, It’s Your Choice: Your Health
Insurance Benefits for 2010. The program explains how the books are organized, where to find
specific information and highlights important factors to consider when choosing a health plan for
2010. Find it on ETF’s Internet site, under the Group Health Insurance menu, at
http://etf.wi.gov/.”
On
page 13, Question and Answer #15, under the Important Note, there are two changes.
1. The first paragraph should end with: “The monthly imputed income amounts vary by
health plan and are provided for either 1 non-tax dependent, or 2 or more non-tax
dependents. These dollar amounts will be adjusted annually. Employees who are
unsure if a person can be claimed as a dependent should consult IRS Publication
501 or a tax advisor.”
2. The following paragraph that starts with the word, “Second”, should read: “Second,
employees may change from single to family coverage to add a newly-
eligible domestic partner or other dependent who does not qualify as a
dependent under IRC Section 152 during the plan year. The additional premium
attributable to the non-qualified dependent will be taxable.”
On
page 14, Question and Answer #17, language regarding full-time students has been removed
as unnecessary under section 1, in the last paragraph. The bullet should read “An unmarried
parent whose only eligible child becomes disabled and thus is again an eligible dependent.
Coverage will be effective the date eligibility was regained.”
On
page 18, the Health Fair Dates & Locations chart for UW-La Crosse lists an incorrect ending
time. The Health fair is open from 10:00 AM to 2:30 PM.
On
page 26, the Health Plan Map incorrectly omitted Arise Health Plan from Lincoln County.
Arise has providers in Lincoln County and should appear as non-qualified.
On
page 28, footnote, language has been added to create a footnote for the Standard Plan at
Plan Tier 3. The footnote reads: “Out-of-state residents assigned to work out-of-state
receive the Standard Plan at a Tier 2 level.”

page 36, the Health Plan Features – At a Glance listing for Health Tradition Health Plan
incorrectly notes that there is a deductible on their dental coverage. No bullet should appear in
the dental deductible column as none applies.
On
page 41, the plan description page for Arise Health Plan incorrectly omitted Lincoln County
from the map. Arise has providers in Lincoln County and should appear as non-qualified.
On
page 46, the plan description page for HealthPartners Health Plan has an error in the phone
number for Referrals and Prior Authorizations. The correct phone number is (800) 883-2177.
On
pages 48 and 49, the plan description pages for Humana Eastern and Humana Western, the
Enrollment Hotline on the page’s banner has been corrected to read (888) 393-6765.
On
page 69, in the Health Plan Report Card section, the title should read "CAHPS Composite
Rating Chart". It was incorrectly labeled "CAHPS Overall Rating Chart".
On
page 83, the Instructions For Completing Health Insurance Application/Change Form should
list the Rel Code for Domestic Partners as 53 and the “Rel Code” for Dependents of
Domestic Partners as 38. It incorrectly transposed the “Rel Codes” for Domestic Partners and
the Dependents of Domestic Partners.
On
pages 85 and 87, Section 2., D., under “Reason”, Three asterisks should follow the
“Domestic Partner” subject to refer members to the footnote, below.
On
pages 86 and 88, Section 8., box title “Date WRS Eligible Employment Began
(MM/DD/CCYY)” should also include “or Graduate Assistant Appointment Began” prior to
(MM/DD/CCYY).
On
page 87, title should read "Duplicate Application for your Records". It was inaccurately
labeled “Second Application”.
